Dance Dance Revolution EXTREME is a music video game developed and published by Konami and released on arcade and PlayStation 2 platforms in 2002.

- Final DanceDanceRevolution arcade game to run on BEMANI System Digital 573 Hardware.
- First DanceDanceRevolution game to introduce the "MARVELOUS!!" timing judgment, which is ranked higher than a "PERFECT!!". It is only used in courses.
- In normal gameplay, the arrow flash will be white instead of yellow if the arrow is hit within the MARVELOUS window.
- First DanceDanceRevolution game with EX Score, which is used for the scoring display when playing CHALLENGE mode (replacing the 100.0% percentage). This alternate scoring scheme would not appear again until X2.
- First Japanese arcade DanceDanceRevolution game to include the BEGINNER difficulty, first seen in Dancing Stage EuroMIX.
- Like in EuroMIX, dancers appear on the screen giving instructions to players on how to step in rhythm. Background animations are turned off.
- Several returning songs with BEGINNER charts first introduced in EuroMIX or its sequel were ported to the game, while songs lacked one were given brand new BEGINNER charts.
- Only DanceDanceRevolution game where BEGINNER charts do not have parameters on the GROOVE RADAR.
- The bitrate of the songs' audio has been reduced to 112 kbps due to disc space limitations.
- The bitrate of the previews was reduced as well, down to 56 kbps mono.
- Background videos have also been reduced in quality to save space.
- Many original videos or unique scenes in videos from songs that appeared in DDRMAX and DDRMAX2 were removed as well due to system limitations.
- On the music select screen, the foot ratings for PARANOIA survivor MAX and The legend of MAX's charts rated Level 10 glow.
- NONSTOP Mode returns, now with difficulty selection: NORMAL or DIFFICULT (which increments one or two difficulty levels higher).
- PLAYER'S BEST courses (1-4, 5-8, 9-12, 13-16, 17-20, and WORST) introduced, featuring the four most or least popular songs selected on the cabinet.
- RANDOM courses also return, which randomly select any four songs and its difficulties in courses.
- The CHALLENGE MODE exclusive songs are now available as regular songs, all but one of them in the DDRMAX2 folder. (HYSTERIA 2001 appears in the beatmania folder.)
- Their banners and backgrounds are also re-colored from the original ones that were seen in DDRMAX2.
- If either player is playing on BEGINNER or LIGHT difficulty, 1st STAGE cannot be failed.
- First DanceDanceRevolution title to include pop'n music, pop'n stage, GuitarFreaks & DrumMania, KEYBOARDMANIA, ParaParaParadise, MAMBO A GO GO, and beatmania III songs.
- Only arcade DanceDanceRevolution game to sort beatmania, beatmania IIDX and beatmania III songs with separate labels.
- First arcade DanceDanceRevolution title to indicate a DanceDanceRevolution arcade original song's game of origin, instead of listing it as a KONAMI original like in previous games.

- Contains the largest on-disc home console version song list of any DanceDanceRevolution title, with 111 songs.
- However, counting available DLC song packs, DanceDanceRevolution ULTRAMIX4 still retains the record for largest overall home console song list with 163 songs.
- Although based on the original arcade version, the NONSTOP MODE course music select is reused from the North America release of DDRMAX2 -DanceDanceRevolution-, as opposed to the original arcade version.
- The READY, HERE WE GO, and CLEARED graphics are taken from DDRMAX2 -DanceDanceRevolution 7thMIX-, instead of the arcade version of EXTREME. The graphics from the original arcade version of EXTREME are used in Dancing Stage Fusion CS (PS1).
- Bit rate quality has increased from 112kbps to 192kbps.
- Background videos also look sharper and cleaner.
- First Japanese DanceDanceRevolution title since DanceDanceRevolution 5thMIX CS with playable dancers in the background.
- Every dancing character from the series up to 5thMIX is unlockable.
- Unlike DDRMAX2 -DanceDanceRevolution-, BEGINNER charts only give a maximum score of 10,000,000, regardless of difficulty level, and have no Groove Radar stats.
- SENORITA (Speedy Mix) is missing from the song list.
- Can unlock everything in DDRMAX CS and DDRMAX2 CS through System Data Support.
- PARANOiA MAX (DIRTY MIX)'s playback sample uses the audio from the CLUB VER. of the song.

- DanceDanceRevolution EXTREME CS (North America) contains many changes to the interface of the game compared to its arcade counterpart. (DDR FESTIVAL and Dancing Stage Fusion CS (Playstation 2) would later use this interface/scoring system.) Here is a list of differences:
- The song wheel interface is completely different from DDRMAX and DDRMAX2, such as the absence of the GROOVE RADAR.
- Scoring system is unlike any other in the series before; songs give you a max score of 7,000,000, with a certain amount of bonus points possible afterward (up to 3 million, for a maximum of 10).
- You must MAX COMBO a song with a high amount of PERFECTs to get a AA. Breaking the combo at any point in the song will only let you get a maximum grade of A, regardless of the rest of your performance in a song.
- Jumps count as one step for combos, similar to pre-DDRMAX installments and all the arcade DanceDanceRevolution games since DanceDanceRevolution SuperNOVA.
- Top and bottom parts of the screen no longer having opaque bars in-game. This allows for more space to see the arrows from below.
- First DanceDanceRevolution title in any region that uses videos for KONAMI original songs when available, eliminating on-screen dancers.
- First CS DanceDanceRevolution game in any region compatible with Sony's EyeToy accessory, allowing access to several minigames and gameplay modes otherwise unplayable.
- First DanceDanceRevolution title on PlayStation 2 with MISSION MODE.
- First North American DanceDanceRevolution title with both NONSTOP and CHALLENGE modes.
- Only DanceDanceRevolution CS title without Dance Play settings; the settings are still in the game, but a cheating device is needed in order for it to be used.
- NONSTOP, CHALLENGE, and EDIT DATA modes all appear on the song wheel.
- TRAINING MODE no longer lets you play songs with slowed down audio when the speed of a song is set lower; it simply mutes the song.
- EXTRA STAGE system from DDRMAX and DDRMAX2 returns, but this time requiring a minimum difficulty of clearing an 8 footer on Heavy.
- Unlike DDRMAX and DDRMAX2, there is no ONE MORE EXTRA/ENCORE EXTRA STAGE.
- Following the title change in the DanceDanceRevolution 5thMIX ORIGINAL SOUNDTRACK album, THEME FROM ENTER THE DRAGON (Revival 2001 Mix) was renamed to THEME FROM ENTER THE DRAGON(notorious mix).
- DO ME (H.I.G.E.O Mix)'s banner is completely different, using some of the background graphics of the song for its banner.
- LOVE LOVE SUGAR's banner is the only banner containing Japanese text without romanization of any kind.
- MEMORIES is unlocked by inputting a command on the main menu (see Trivia section below for the unlock method).
- The ROULETTE and RANDOM music (which is also used in DanceDanceRevolution Party Collection) is a sped-up version of the following songs in a loop:
- B4U, DROP THE BOMB, AM-3P, STILL IN MY HEART, CAN'T STOP FALLIN' IN LOVE, CELEBRATE NITE, Try 2 Luv. U.
- Only DanceDanceRevolution CS title to win an award, for Best Game Soundtrack in the 2005 MTV VMAs.
- Although INFORMATION credits the in-house licensed covers as being from Karaoke Revolution, they are instead remixes of the covers featured in that game.
